This hook allows you to ask a question to your clients right before they get the EPP code using a jotform.com form.
Once they fill the question they will be redirected to the client area where the EPP code is shown.
RetiredThe first thing we have to do is to go on jotform.com to create the form and retrieve the id to be able to configure the hook. You can use jotform.com with a free account. Create a free account, then search the template named "Auth-Code/EPP WHMCS request exit form" and click on use template.
here is what you have to do to adapt the template to your needs:
Download the Hook and copy the file in WHMCS/includes/hooks.
This plugin requires the Custom module to manage hooks settings which you can download for free from our website. You will need to add the setting "jotform_authcode_id" and the form id as the value.
Test it, to make sure all the redirects are working well and you're done.

jotform deleted it, I created it again.
